Sacramental Records Request

In addition to Immaculate Conception’s own records (1855-present), the office maintains the sacramental records of the following closed parishes:

Mary Help of Christians (East 12th Street, between 1st Ave & Ave. A) 1906-2007

St. Mary Magdalene (E. 17th Street between Avenues B & C) 1873-1945

St. Ann (E. 12th Street between 3rd & 4th Avenues) 1853-2003 *

To request a copy of a sacramental certificate, please make the request in writing via the contact form below, including as much information as possible (e.g. the date of birth/date of baptism, date of marriage, names of godparents/witnesses).

Please allow two weeks for processing.

The fee per certificate is $10.00. The fee for genealogical research for records prior to 1930 is $25.00 per name.
